Transitions Coordinator – This full-time position is based in FREED’s business office in Grass Valley.
FREED is seeking qualified candidates for the position of Transitions Coordinator. The Transition Coordinator facilitates service coordination for those residents who choose to relocate from an inpatient facility to community living. Transition coordinators, in cooperation with facility discharge planning staff, link the individual to necessary services and supports for their successful discharge and return to community living. EOE, Individuals with lived experience of disabilities and/or bilingual individuals are strongly encouraged to apply.

Assistive Technology Advocate – This full-time position is based in FREED’s business office in Yuba City.
FREED is seeking qualified candidates for the position of Assistive Technology (AT) Advocate. The AT Advocate will carry out duties related to FREED’s AT programs and services including providing AT information and referral, outreach, education, technical assistance, and training. The AT Advocate provides staffing for the Equipment Reuse program to support consumer AT related goals for independent living. The Assistive Technology Advocate actively participates in the statewide Ability Tools Network and carries out all duties defined in the annual AB204 AT plan.
