This team provides general homeless outreach to both Sutter and Yuba Counties intending to build relationships and increase engagement in helpful services while reducing public and private property crimes. HEART collaborates with other street outreach teams and community members.
Entry point for referrals to shelter and housing services, laundry, showers, and other support available for families and individuals experiencing homelessness.
Complete medical, dental, vision, behavioral health, discount pharmacy, HIV/STI testing and treatment, PrEP services, and case management. Medi-Cal, Medicare, Tri-Care, and most forms of insurance accepted. Sliding fee scale available.
